How can I receive a copy of my medical record? Once the Health Information Department receives your written request, we can mail your records or you can make arrangements to pick up the records at the business office at the John Nolen Drive location. Is there a cost for receiving a copy of my medical record? Turville Bay does not charge patients for paper copies of their medical record, but there is a $5 charge per personal copy of MRI images on CD. Turville Bay also charges a fee when releasing medical records to a requesting third party, such as an attorney. Who do I contact to request a copy of my medical record if my MRI was done at UnityPoint Health-Meriter Hospital or SSM Health St. Mary's Hospital-Madison location? Regardless of where your MRI imaging was performed, all release requests are processed in the Health Information Department at our John Nolen Drive business office. Turville Bay requires a written request from the patient. However, medical records for a minor can be requested by a legal guardian. How long does it take to process my request for records? Approximately 3 to 4 business days. In most cases, medical record requests can be provided by the next business day. However, the Medical Imaging Department requires 48 hours notice for CD requests. I have an appointment with another physician to review my Medical Records. Can I stop by the business office to pick up the records and/or the images? Although the Health Information Department aims to respond to release requests in a timely manner, we ask that you please call as far ahead as possible when requesting records. Health Information staff can fax a copy of the results to the physician and in some cases, we may be able to send the images to the physician electronically. In cases where a CD of the images is needed, our Medical Imaging Department requests 48 hours notice to process the request. How long after my MRI appointment will my referring physician receive a copy of the results? The referring physician should receive a faxed copy or have access to an electronic copy of your MRI results within 3 to 5 business days. If the physician has not received a copy, please contact the Health Information Department. Will my physician have access to view my results or MRI images electronically? Not all physicians can view Turville Bay MRI images electronically. Physicians who have access to UnityPoint Health-Meriter or SSM Health-St. Mary’s Hospital Madison EPIC system may have the rights to view your images and results, depending on where your exam was read.
